Fat Grafting

Fat grafting techniques have been improved over the last 10 years and have become a more common procedure for recontouring or augmenting desired areas like buttocks, breasts and face. Fat grafting transfers fat from one area of the body to another. The outpatient procedure is used to rebuild or rebalance parts of the body that have naturally lost volume due to aging. During the procedure, fat is often removed from the:
  • Abdomen
  • Hips
  • Thighs
And transferred to the:
  • Face
  • Breasts
  • Buttocks
  • Hands
Fat grafting is a safe and well-tolerated procedure that does not require incisions or stitches. It is performed under local anesthesia applied to the body areas being sculpted. A Temple Plastic Surgeon uses a tiny cannula to remove unwanted fat. The fat cells are kept alive, centrifuged to obtain the best cells and then carefully injected into the recipient sites.

Using an outpatient technique called liposuction (commonly referred to as "Lipo"), Temple Plastic Surgeons can transform the body's contour in specific areas, enhancing your body's shape and proportion by removing excess fat deposits. Liposuction techniques can reduce fat deposits localized in the following areas:
  • Abdomen and waist
  • Hips and buttocks
  • Thighs
  • Inner knee
  • Chest and back rolls
  • Neck
Liposuction is performed through small, inconspicuous incisions, using a narrow hollow tube, called a cannula. The cannula is inserted into the incisions to loosen excess fat. The cannula has a syringe or surgical vacuum attached, which suctions out the fat.

Temple Plastic Surgery offers traditional (tumescent) liposuction and SmartLipo, laser-assisted liposuction which helps tighten the skin right after the liposuction. SmartLipo is generally used when there is less fat to remove.

If you are a healthy, nonsmoking adult within 30% of your ideal weight with firm, elastic skin and good muscle tone, you are a good candidate for liposuction. It is not a treatment for obesity nor is it an effective treatment for cellulite or saggy, damaged skin.

Tummy Tuck

Tummy tuck surgery (abdominoplasty) is an outpatient procedure designed to achieve a smoother and firmer abdomen by removing excess fat and skin and restoring stretched out abdominal wall or weakened muscles. A regimen of hundreds of sit-ups a day may not be enough to repair the damage from multiple pregnancies. When exercise and weight control do not produce your desired flat, well-toned abdomen, then a tummy tuck may be for you.

A tummy tuck is not a substitute for weight loss or a regular exercise program. Tummy tuck results, while permanent, can be diminished if your weight fluctuates significantly. That is why you should postpone a tummy tuck if your plans include losing substantial weight or future pregnancies.
The most common causes of a protruding or sagging abdomen include:
  • Pregnancy (especially twins or multiple pregnancies)
  • Significant weight fluctuations
  • Heredity
  • Aging
  • Prior surgery
Full tummy tuck surgery will leave a horizontally-oriented incision in the area hidden below the bikini line. The amount of excess skin to be removed determines the shape and length of the incision. If a vertical abdominal scar already exists or if a patient wants to remove side rows of excess fat and skin, then a "Corset" abdominoplasty may be performed for the best contour. For tummy tuck scars to maximally fade away, it can take anywhere from several months to a year.

It’s important to follow your surgeon's instructions for a smooth and successful post-surgical recovery, which will include dressings for your incisions, temporary small tubes to drain excess blood or fluid, and a compression garment to reduce swelling and provide support for your healing abdomen. Your doctor will discuss all of this before your surgery.

Mommy Makeover

For women who want to regain a more youthful figure after having children, a so-called ‘Mommy Makeover’ combines both a tummy tuck with a breast lift, and may also include other procedures, including liposuction. The outpatient procedure results in a firmer, trimmer abdomen and more youthful looking breasts.

Male Breast Reduction

Gynecomastia, or over developed or enlarged breasts in males, can occur at any age. The condition can be caused by hormonal changes, heredity, obesity or prescription medications or other drugs, including steroids.

The condition can affect self-confidence and result in emotional discomfort and embarrassment. Men whose breast sizes have stabilized may be candidates for breast-reduction surgery. Temple Plastic Surgeons can reduce the breast size and flatten and enhance the chest contours. The outpatient procedure, which involves either IV sedation or general sedation, can involve several techniques.

If the cause of the condition is excess fatty tissue, your surgeon will utilize liposuction by making several small incisions and then inserting a thin, hollow tube to vacuum out the excess fat. Particularly in severe cases of enlarged breasts, glandular breast tissue or excess skin may need to be removed. In such cases, the Temple surgeon  will make larger excisions in order to surgically remove the excess tissue and skin and reposition and re-size the areola—the dark skin surrounding the nipple.

Body Contouring After Major Weight Loss

For many people, the satisfaction of losing weight is undermined by the appearance of loose skin around the abdomen, arms and thighs. A side effect of massive weight change—100 pounds or more—the excess skin has lost its elasticity due to excessive stretching. The skin can also be weighed down by stubborn fat pockets that refuse to respond to diet or exercise.

Aside from obvious cosmetic concerns, loose skin after weight loss can cause chafing, irritation, infections, cysts and body pain. Such problems may occur after any substantial weight loss, and are not limited to patients who underwent bariatric surgery.

Temple provides body contouring surgery as a means of reshaping the skin to conform to the body's new shape. To accomplish this, our fellowship-trained plastic and reconstructive surgeons offer a series of outpatient procedures for removing excess skin and fat and strengthening the underlying tissues.
  • Tummy tuck: Temple offers a procedure called abdominoplasty, or tummy tuck. During this procedure, a Temple reconstructive surgeon removes excess skin and fat from the abdomen while rebuilding any weakened or separated abdominal muscles. In a related procedure called panniculectomy, the surgeon removes any excess abdominal skin hanging below the belly button.
  • Arm lifts: To achieve a tighter contour around the arms, Temple offers arm lift surgery, or brachioplasty. During this procedure, a Temple surgeon removes excess fat and hanging skin from the arm. The incision required for a brachioplasty extends from the armpit to the elbow.
  • Upper leg lifts: Temple offers surgery to remove excess skin and fat from the thighs, resulting in a smoother leg contour and tighter lower body shape. The incision starts at the groin and can extend around the hip and as far down as the knee.
  • Butt lift: Temple offers surgery to remove excess skin and fat from the buttocks. Two surgical techniques are available, with incisions being made either in the crease of the buttocks or between the buttocks and the lower back. Patients with excess fat may also simultaneously undergo liposuction treatment. For patients who have lost volume in this area, butt augmentation—which involves implants, fat transfers or a combination of the two—is also an option.
The dramatic results of body contouring surgery are visible almost immediately, though it may take a year or two to see the final outcome. All body contouring procedures will leave a visible scar. Temple's plastic and reconstructive surgeons are highly skilled at minimizing scarring, but it is impossible to eliminate it completely.

It is important to note that most insurance companies only cover abdominoplasty and panniculectomy after weight loss. Most, but not all, insurance companies consider arm, thigh and butt lifts to be elective post-weight loss procedures.
